Healthcare Support
CHSGa provides reasonable modifications and free aids and services to people
with disabilities to communicate effectively with us, such as:
- Qualified sign language interpreters;
- Written information in other formats, including large print, audio, accessible electronic formats, and other formats.
CHSGa also provides free language services to people whose primary language
is not English, such as:
- Qualified interpreters;
- Information written in other languages.
If you need these services, please contact the Administrator of the facility,
agency, or office where you are located.
Grievance
If you believe that CHSGa or its affiliate(s) have failed to provide these
services or discriminated in another way on the basis of race, color, national
origin, sex, age, or disability, you can file a grievance with the Civil Rights
Coordinator – Section 1557 (CRC).
You can file a grievance in person, by mail, or by email. If you need help
filing a grievance, the Administrator of the facility, agency, or office where
you are located or the Civil Rights Coordinator – Section 1557 (CRC) is available
to help you.
File a Civil Rights Complaint
You can also file a civil rights complaint with the U.S. Department of Health
and Human Services, Office for Civil Rights, through their electronic Complaint
Portal, available on the HHS Office for Civil Rights website, or by mail or phone.
